If your dad has pellagra, niacin should help. This is a problem endemic to Europe and a diagnosis originally identified by a Spaniard. I lost the first question you had written in Spanish. Your profile does not say where you are.

It seems if you are in Europe and the docs have not been able to figure out what it is or how to treat it I would question the diagnosis of pellagra or change doctors, It is a nutritional deficiency but there are a few ways to get it.
