Well, one really can pick up the odd gem here on SD. For 'of course' I only ever hear 'por supuesto' or 'claro que sí ' here in Spain. Now I know 'cómo no'. Just shows you never stop learning.

"Cómo no" can take more meanings,

The first one is which you've pointed, this is very polite.

¿Puedes decirme qué hora es?, - ¡Cómo no! Las tres y media. Could you tell me what time is it? Of course! It's half past three.

Another meaning sounds quite poetic:

¡Cómo no recordarte! How could I have forgotten you!

It also can mean "Why"

¿Cómo no fuiste ayer a la fiesta? Why not did you go yesterday to the party?

If the "que" is included, the meaning changes significantly:

No, eso no es así. -¿Cómo que no? That is not true. - What do you mean?
