Typically, a slang greeting used to say "What's up?" "What's happening?" or "What's going on?" More literally it may be translated into "What has passed?" to ask "In the time that has passed (since I last saw you), what have you done?"

Some common replies would be "Nada muncho." "Lo mismo." for "Nothing much" or "Much of the same"

Ciao!

¿Qué pasa?

It depends on the context. As a greeting it works a bit like "What's up?" or "How do you do?" (but less formal), which is not a real question and it can be answered using the same words, eg. ¿Qué hay? If something unusual seems to be happening, but it means "What's the matter?"

¿Qué pasa? if I am not mistaken it means "what's happening" I use to hear it a lot in the seventies. It all depends on the indivisual, as far what the answer will be . Puedes decir nada o nada de particular or whatever is going on at that time.
