"¿Desde cuándo se entiende con ella?" How long have you been "having an understanding" with her? As Margaret correctly surmises, this is a rather formal and old-fashioned way to ask the question that you already understood.

Could it be 'se entiende'? - Sounds old fashioned to me - like 'how long since they came to an agreement (to marry) to be engaged, or whatever' .

Well, he definitely said "how long have you felt/been in love with her" because I've seen this show in English. But I wonder about the form.
