Is the phrase "que tal, mi amiga" common heard in Spanish?

Yes, it simply means, "What's up, my friend?" (Amiga would be a female friend.)
I would never say such a thing (I'm from Spain) unless I intended to sound funny or ironical with that "mi amiga".
"que tal, mi amiga"
We would , if at all, say that the other way round, in Spain, that is, I know in other countries it is common to use it this way
¿Qué tal, amiga mía?
The Qué tal part of your question is just a greeting saying How are things with you or words to that affect, I don't think it means "what's up", that would be "qué pasa" as for the complete phrase I would take Heidita's answer.
"Qué tal mi amiga/o"
"Cómo le va mi amiga/o!?"
"Cómo anda mi amiga/o?"
are all quite common phases in Argentina between old people, specially when you want to sound polite.
