How do you say 'Required for calback"
Greetings: I'm preparing a form for obtaining information.
I need their phone number if they want me to call them back, so I want to say simply.
"required for callback"
How? Thanks!
"Necesito para volver a llamar"
Is the above correct?

If you do not mind an off topic remark:
In the description portion above you are using the polite, formal tone. (le, usted, )
Down where all of your commands are you are mixing formal & informal commands. [(escoja (formal), envía (informal), borra (informal)]
You use su rather than tu and then use an informal command.
Did the buttons images come with the commands already on them?

Rogamos dejen su número para devolverles la llamada.
I need a short version...thanks.. How about just ('Para devolverles la llamada).
Now that I am seeing the picture, quentin , I do wonder how you do that, well, never mind:

If you say return your call they will think they have to call you first.
