My friends kids are calling her "viejita", and she's not happy about it! What does it mean?

Well it depends on the place, I think, in many countries is not bad, I suspect. I my case, I would emphasize the “ita” part of this word by emphasizing love and not obviously age (“ita” is also used on children). I guess it depends also on “how” to use it (the tone of the voice). It could also depend on the person too, people with big ego may never like it at all. But don’t try to understand a language imposing your culture, impose love on you and it will be easier to understand other cultures. Still, one have to be careful because there will be always "babies" up there who will get offended with just about anything (OK that link was "selfish" but the message linked is not smile.

In Mexico almost every husband calls his wife viejita, or just plain vieja at some time. It is a term of endearent. You will also her flaca, gorda, etc. in Mexico

It is the diminutive of "vieja" (old woman), but this "-ita", which rather turning the word into "little old woman", it makes it sound more affectionate. Still, the word "vieja" is not the nicest word to address a woman, even with that "-ita".
