Does ¿pasa algo? mean "is something happening" or "is something wrong"?

How I see these - ¿qué te pasa? - what's wrong with you? - no pasa nada - it's all good, it's all cool, no prob bob, nothing is going on

So should "pasa algo" mean "is something happening" or be more like "is something wrong" like with "qué te pasa"? Gracias.

I think it is more "is something wrong?" Literally it means "is something happening?", but it is a spanish expression to ask you if there's something wrong. In Portuguese we use the same expression.

Both. It depends on the context and the intonation. It could also be used in an aggressive and defiant way, and probably in other ways too.
