Hi, Chica. I think it depends on where you are.

If you're in a location where Kleenex is sold in every grocery store, then many people will say Kleenex. However, if you're somewhere where it's not so popular, you'd hear "pañuelo" more.

In English, I say "tissue" and in Spanish I say "pañuelo" no matter where I am and I'm understood.

I think it is based on the region you are from, here in the US you would probably hear Kleenex more but the common or proper one is pañuelo.

I can only tell you that my Grandmother's friend always said pañuelo. Kleenex is actually a brand name of tissue. I know we often say Kleenex when we need a tissue but there are many more brands of tissue.

There might be another word that is more commonly used for handkerchief and perhaps others will post with their suggestions.
