"Podría usted me ayudar?" [WRONG]

¿Me Podría usted ayudar? [CORRECT]

The rule is simple: pronouns like "me" should appear in general right before the verb (there should be no other words in between). The only exception is when they attach themselves to a verb, which can only happen with certain specific constructions having two verbs, with the second one being a gerundio (pretty much like present participle) or an infinitivo, and with positive imperative forms. In the past they appeared most often, but not nowadays.
