¿Cómo se llama (usted)? Formal - What is your name?

¿Cómo se llama (él / ella)? What is his / her name?

¿Cómo te llamas? Informal - What is your name?

¿Cómo te llama? means what does he or she call you whereas ¿Cómo te llamas? means what do you call yourself meaning what is your name? that is the normal way of asking someone their name in the familiar form. ¿Cómo se llama? is what is your name in ustd form like to a stranger or older person someone that you shouldn't be over familiar with etc.
