Which is better.
mi cabeza es duele, or tengo un dolor de cabeza. I have a headache but some of my friends think I have bumped my head.

I would say, "Me duele la cabeza." I hope this helps. ![]()
I would say me duele la cabeza
mi cabeza es duele doesn't make sense to me. However, you could use your second example.
You can use "Tengo un dolor de cabeza". I hope you feel better soon. ![]()
Since nobody else mentioned it explicitly, "mi cabeza es duele" is totally unnatural. Someone might be able to figure out what you meant but no Spanish speaker would ever say this.
I have a headache but some of my friends think I have bumped my head.
My friends have been thinking that about me for years ![]()
I would say:
Tengo un dolor de cabeza = I have a headache
Me duele la cabeza = My head hurts
It's a subtle difference, but maybe that's why they think you bumped your head.
Me duele la cabaza.
