I'm still typing out this health questionnaire..should I ask the questions in usted form or "tu" form:
ejemplo- Bebes alcohol? o Bebe usted alcohol?
¿Estas preocupado por la cantidad que bebes? o ¿Esta usted preocupado por la cantidad que bebes?

Where are you based? If Spain, I'd suggest going for the formal as that seems to be a convention with "official" type documents.
