Typing out a questionnaire in spanish, was wondering if someone could help me translate some simple frases . Thanks a bunch.
- How to say "partnered" in Spanish(when asked about your estado civil)
-to say "previous or reffering doctor" is it right to say "doctor anterior o referido" ?
- The immunization i MMR(measles, mumps, rubella)- I know in spanish measles is Sarampión, mumps is paperas, rubella is rubeola so in spanish is the abbreviation going to be SPR?

Estado civil: pareja de hecho
This is what I would have said, but it seems that legally this is not contemplated:
c) Respecto de la situación familiar, se señalan los estados de soltero, casado, judicialmente separado, divorciado y viudo,
So that would be : soltero
-to say "previous or reffering doctor" is it right to say "doctor anterior o referido" ?
I would say: médico de cabecera anterior o ordenante del traspaso
