According to this site it is "segundo nombre". I would be careful using that without making its meaning clear, however, because middle names aren't a Spanish custom, and since they use two surnames (apellidos) or more in their complete name, you might find some confusion.

I like the alternate definition of a "2nd first name" or segundo nombre de pila. (baptism)

When I lived in the South (USA) everyone was called Billy Bob or Norma Jean, etc. I wonder how hispanoparlantes view that tradition?
