Here it is; The list!

Top 5: How are you? -- ¿Cómo es usted?

Top 4: Good night -- Buenas noches

Top 3: Good day to you -- El día bueno a usted

Top 2: Where are you from? -- ¿Dónde está usted de?

People really want to know this: Nice day to all of you! -- ¡El día agradable a todos ustedes!

(formal) ¿Cómo está usted? (informal) ¿Cómo estás?

Buenas noches.

Que tengas un buen día. (I do not hear that much...)

(formal) ¿De dónde es usted? (informal) ¿De dónde eres?


Other phrases you can say besides "que tengas un buen día"...

Que le vaya bien... literally... that it goes well for you. (formal)

Que te vaya bien...(same meaning, informal)

Que te lo pases bien... Wishing someone well, that they have a good time ... with plans they may have.(informal)

Que se lo pase bien... (same meaning, formal)

Nothing wrong with ¿Cómo es usted? but I doubt that it is more commonly asked than ¿Cómo está usted?
