usted es mi un y solamente
you are my one and only
(If you put this into a greeting card, it is perfect)

"tú eres mi único"
I think "usted" is too formal to use with "the one and only" in Spain. In Argentina it could be "vos sois mi único" but i'm not sure if that is correct.
