Diminutives are sure to be a puzzle for many English speakers. They are very versatile tools in Spanish, though.
In this case,"a puntito" just means I was "really-really this close to escaping", as opposed to "a punto", which is merely "on the verge".
I just thought to add: In English it might be a bit like "Ever so close".
