I believe it is something along the lines of a play on words...Literally "He doesn't even knock at the door because he would get it out of tune." It makes no sense as a play on words in English... "Tocar a la puerta" is a way to say "to knock at the door". And - as you observed - "tocar un instrumento" is to play an instrument. "Afinar" is "to tune an instrument" and "desafinar" refers to something being "out of tune". It is a cute play on words in Spanish, but in English the humor doesn't translate...
