Que fue lo que pasó
What was what happened? = what happened? Why not just say "What happened" = "Que pasó?"

A skinny nerd beat up the class bully into a bloody pulp, and the other bullies are standing around. One of them has a cigarette in his mouth and he opens his mouth in surprise, the cigarette drops to the ground...and he says "¿qué fue lo que pasó?" So I'm gonna assume it means "What in the heck just happened?!?"
Assuming there is a tild overe that qué to make it a question word, it's like "What it was that happened" or "What was it that happened?".
Think of when you'd say that in English....
"That's not what happened!" (No fue lo que pasó.)
"Then, what was it that happened??) (Entonces, ¿qué fue lo que pasó?)
