Is Sra. Gomez a person asking for an object (a salad) or is someone asking Sra. Gomez. I hope my question can help you figure it out.
You have written what someone might say, indeed, but not a complete sentence. Having the complete sentence with even the unexpressed thoughts in words would make it easier - as one says in grammar - to diagram.
