It's common to say something like "Ando bien de salud" , "Ando mal de salud" ("I am in good health" or "I am in poor health"). To say "fair health" you could say "Ando bastante bien de salud" . If you want to use "La salud" as the subject, you can say "Mi salud es bastante buena".
