If someone did not have insurance and was listed as "self-pay", would you say, "Le voy a apuntar de pagador de su propia bolsa?" Any other suggestions?

That sounds odd. I think the correct translation will depend on the country.

In Argentina, by example, we would say "sin cobertura médica prepaga".

I assume that we're discussing Health insurance as property insurance usually requires some type of bond or other collateral.
