Whenever I heard anyone say "¿Qué te pasa?" it was as if they were asking "What is wrong with you?" (like they were a child causing trouble) or "What's wrong?".

"¿Qué pasa?" means "What's up?"

"¿Que pasa?" is more likely to mean "What is going on with you?" or "What is wrong with you?" even "What is bothering you?" But you don't need the te.

The "te" makes a difference in the meaning. See Nathaniel's reply. - samdie, Feb 8, 2010
It is interesting how usage varies from person to person or region to region....this is the only difference here. In my many conversations with my Mexican friends they have seldom...if ever used te in that phrase. for them it is all voice connotation. - renaerules, Feb 8, 2010
