To ask how a friend feels you would ask:
¿Cómo te sientes? (That 'te' is important because without it, it would sound like you are asking the person how he is feeling some object, like the bed sheets, or the cup in his hand.

Chaparito is correct, and the grammatical explanation for that is called the reflexive. After a surgery, the "feel" stays within the person's being or body, and feeling anything outside of their body would be called the transitive. There is also the formal, when dealing with older people or strangers you don't know, "se siente". Another common verb is "sentarse" to sit, so make sure not to say "se sienta" or "te sientas" which means "you sit". One little letter and the word meaning changes
