Yes, it means now. Many Spanish phrases using now, however, use ya or actual.

Ahora, María está arreglando su habitación. Now, Mary is tidying her room.

¡Hágala, ahora mismo! Do it right now!

Yes it does mean now eg.Antes estaba gordo ahora estoy delgado. I used to be fat now I am slim.
