Combining direct and indirect object pronouns

When combining direct and indirect object pronouns into the same sentence, when do you put them on the end of the verb and when do you put them before? (i.e. damelo vs. me lo da)

After: positive commands

Before: negative commands

Either: gerunds, infinitives

In all other tenses and uses, the pronouns go before.
