Here there is a link about subjunctive. I just looked through it very quickly but it look a good book. I have only seen a mistake: "durmaamos" instead of "durmamos". It can be downloaded.
And this other link, it is somewhat similar to the first one but it has more explanations in English.
And this one is a type of summary although it cannot be downloaded.
