How would you specifically define a collared shirt?

I understand that "camisa" typically implies a collared, buttoned shirt, but for the sake of a dress code, would "camisa con cuello" work? Or is that repetitious?

To be very specific for a dress code, yes, you should define what type of shirt is acceptable under the dress code. "Camisa con cuello" sounds good to me.

How about: Camisa con corbata? Wearing a tie is formal and you couldn't wear a tie with a collarless shirt. You could, but it would look silly!

I think in English, we say "collar and tie" for formal wear.

Well, I would go with that otherwise, but the tie wasn't implied in the list. Only a collared shirt and slacks. I assume that would also include polo shirts.
