El cuidadano = citizen. Soy un cuidadano de europa = I am a european citizen even if I lived in a small village so it is not exclusive to city dwellers, the name for the city dweller could depend on the city they live in for example Liverpool = Liverpudlian Manchester= Mancunian, Birmingham= Brummie etc.

A cuidad is a city. A cuidadno is a person who lives in city.
