I guess my question is, when a native speaker hears Nevera what is the picture in their mind. Same with Refrigerador? Is there a difference like nevera is more like an icebox of old while refrigerador is more like a modern mechanical/electrical? Or, perhaps it is a regional difference or maybe there is no difference at all.

They can both mean frigorífico, so in that regard they're synonyms.
