Hi Michelle, I was in the same boat, I was scared and embarrassed to use spanish in front of spanish speakers. But the only way to learn is to practise! Just go for it! You will be surprised at how well you will do!
Michelle, why not try one of the live conversation Skype lessons? They cost about $10 an hour and the teachers are wonderful.

You are way ahead of most everyone learning Spanish if you have a regular opportunity to speak. If you are in a Spanish-speaking country, you might be embarrassed, you might be afraid to speak, but you will just practice as a matter of course and soon enough, what you see that you can communicate and that its fun to be understood, you won't be afraid anymore.
Of course that is not what you asked. You asked for advice on how to start talking. And for that maybe you just need to be sure you are always out and about. Ride a train...(probably best in second class). Ride somewhere, anywhere so long as the trip is long distance. People will probably want to talk along the way. Just don't break down and let them speak English with you.
