Soldiers refer to them as "Dear John" letters. It a letter saying that "she" is ending the relationship. I believe now that we have so many women in the military that they now also have a "Dear Jane"" usage.
I would call "kiss off" an invective epithet. In more general usage than in a Dear John/Jane letter it is similar to saying "Get lost"

Well it´s not vulgar (nor is it a euphemism for something vulgar) but it does suggest a parting (albeit with a kiss) that was intended to be final (and was not filled with regret). Possibly "Lo despedió para siempre con un beso frio (que faltaba de cariño)."
