The 2nd example uses "vitió" is this correct? I cannot find this conjugation anywhere. I think it should be "vertió"; maybe "vertido" but I don't think so. No big deal just curious.

Verter (ver-terr') to spill; to pour out, to empty; to make; to translate
Examples:
Los ríos vierten en el mar. - Rivers empty into the sea.
La fábrica vitió residuos tóxicos en el lago. - The factory spilled toxic residue into the lake.

La fábrica vertió is correct. "Vitió" or perhaps "Virtió" are not correct .

You are right: the 2nd sentence should have been:

La fábrica vertió residuos tóxicos en el lago.
