Personally, I would say:

Aprieta el tornillo, está (flojo/suelto). Tighten the screw, it is loose.

But I am not a native speaker. Await other input.

I agree with Bosque.

You can use screw too with the meaning of crazy with the following expression.

¡¡ Has perdido un tornillo!! .literally .You has lost an screw .it means that you are crazy
