When you ask día you usually means what day of the week.
¿Qué día es hoy?. Today is Jueves, or Sábado.
La fecha means usually the number of the day and the month (the year is understood).
¿Qué fecha es hoy?. Hoy es 25 de enero (del 2010).
You do not ask both questions at the same time. Sounds bit weird to me.
