An/The all-American boy" is sort of a combination of the ideal and the typical American boy (one who embodies the traits/characteristics [for better or worse] of the American culture). The phrase is probably related to the nation-wide competitions in sports (at the high-school level) which work their way up from all-state to regional and finally all-American. (Since I'm not big on sports, I may have muddled the details about this). For "all" you could substitute "prototypical" / "quintessential" / "paradigmatic" (except that these are not frequently used words).

Note: "the all-American boy" does not mean "all American boys".
