There's lots of disagreements and "I don't know" answers on this one.
Here's my understanding.
To help out - generally to help somebody by doing whatever you can think of doing.
To help - specifically to help somebody do a specific thing that they need help with.
Examples:
You can help me fix the car. You know more about how the engine works.
You can help out while I fix the car. Organize these tools so I can find which one I need faster.
Not too sure about the Spanish interpretation here.
Would you mind helping me? Esta es una pregunta que invita a la persona a pensar en ayudarte.
Would you help me out? Esta pregunta se lo pregunta a la persona directamente que te ayuda.

There is no difference. that I know of. 'Help me out" is just a colloquial way of asking for help. If there is any difference it's so slight it's not worth talking about.

La primera quiere decir "te molestaría ayudarme?" y la segunda más "me ayudarías?" Ojalá que te haya ayudado. No sé explicarlo bien haha.
You can't say "Would you helping me out."
Would you mind helping me? - ¿Me puedes ayudar?
You can say:
Would you help me out? - ¿Me ayudarías?
There is no real difference in meaning between would you mind helping me and would you mind helping me out. It is a matter of preference.
