I have often heard the expression: Puta Madre! Self explanitory, I believe.
Erm...yes. It's mainly used as the word "bitch", but is also a common exclamatory word (i.e. you miss a hit in baseball, "puta!") But yes, it is generally offensive, just like the English equivalent.
