For example, why is "vaca" spelled with a "v" and not a "b".

Encontré información aquí... Sí hay reglas y cuando las personas las confunden es porque no saben (o no han estudiado) ortografía muy bien. Es igual que en inglés cuando la gente se confunde con conjuntos letras que tienen sonidos iguales...

I found information here... The are rules and when people confuse them it is because they don't know (or haven't studied) spelling very well. It is the same as in English when people get confused by groups of letters that have the same sounds.

Thre is no rule (which is why so many Spanish-speakers confuse them). The only guide is the history of the language.

Its not a rule per say, but about annunciation. "Vaca" (cow) sounds like "Baca" when spoken authentically, and thats "just the way it is". Similar to why we use a "y" sound with llamar or other similar words.
