Whats the most accurate translation for "In spite"
A pesar de haber vivido en los Estados Unidos por mas de 20 años Maria todavia no tiene buena pronunciacion en ingles.

Actually, you have it right.
To wit: a pesar de translates to "despite", and despite has "spite" as its root. The phrase "in spite of" means the same thing as the word "despite". So a direct translation of one is also most likely a direct translation of the other.
On a side note, I think you should say más que instead of más de.
