Assuming it's someone you know, I would use the informal form. The formal form would be more for a stranger or your boss or the president or something. "Estuviste muy buena, como siempre"

Como siempre, qué bello te apareciste.

"Usted miraba linda como siempre"

(You looked pretty as always.)


I would use the formal form and for sure the imperfect tense. I am assuming this is said to a female.
