Don't practice until you get it right. Practice until you can't do it wrong. Practice doesn't make perfect. Perfect practice makes perfect.

Practice reading passages (from a newspaper, for example) aloud. Read each one over until you can say it fluidly. . .eventually this will be the first time. Then practice saying it more rapidly if that is what you want, but don't sacrifice clarity for speed.

Whenever you want to learn how do something with speed, you have to practice. Practice definitely makes perfect. Like the above answers, immerse yourself in the language and read books, passages, etc. Practice saying it over and over - especially if it's a word or phrase that seems difficult. Both accuracy and clarity are more important than speed because what's the use in being able to say something quick if it's not even right? Before you know it, it'll just roll off your tongue without you even thinking about it.
