Question

When talking about travelling in a sentence like "We took the train with destination to Milan -for example", I translated as follows

"Coger el tren con destino a Milan"

I've a doubt: is the word destino right for this context and what about the preposition "a"?

Why not just say "destinado a Milan"?

According to this definition, you said it correctly, however.

I've also heard that "coger" is to be avoided if at all possible.

I think so - here are some examples from the dictionary on Spanishdict.com:

el tren con destino a La Paz -> the train for La Paz, the La Paz train

un vuelo con destino a… -> a flight to…

pasajeros con destino a Chicago, embarquen por puerta 6 -> passengers flying to Chicago, please board at gate 6

Only the verb "coger" should be conjugated ----> cogimos
