For my homework it is asking me to fill in the chart below for the verb ser?

This is probably a conjugation chart. If there is a chart available to you in the question, fill in the missing words. Otherwise, make a chart similar to the one here. You should probably use the present tense in the indicative mood (the one in the top left corner of the link).
Most likely it wants you to conjugate the verb ser. It could be many forms: Preterit, Imperfect,Present... etc. Conjugating a verb means naming its yo, tu, nosotros,usted, and ellos form. Here is the present conjugation of ser for example.
